What is Tinnitus?

As tinnitus is mostly a subjective phenomenon, it is tricky to measure using objective tests,eg in comparison with noise of known frequency and power, as in an audiometric test. The condition is often rated scientifically on a simple scale from "slight" to "catastrophic" according to the practical problems it imposes, such as interference with sleep, quiet activities, and standard routine daily activities.
Tinnitus is common; about one in five folk between 55 and 65 years old report symptoms on a general health test, and 11.8% on more detailed tinnitus-specific questionnaires.

Objective tinnitus
In some cases, a clinician can understand an honest to goodness sound springing from the patient's ears. This is called objective tinnitus. Objective tinnitus can arise from muscle cramps that cause clicks or crackling round the middle ear. Some people experience a sound that beats in time with the heartbeat (pulsatile tinnitus, or vascular tinnitus).
Pulsatile tinnitus is generally objective in nature, springing from altered blood flow or increased blood turbulence close to the ear but it could also arise as a subjective phenomenon from a raised awareness of blood flow in the ear. Infrequently, pulsatile tinnitus could be a indication of potentially dangerous conditions like carotid artery aneurysm or carotid artery dissection. Pulsatile tinnitus may also indicate vasculitis or even more specifically, giant cell osseous rheumatism. Pulsatile tinnitus can also be an indication of idiopathic intracranial high blood pressure.

Subjective tinnitus
Subjective tinnitus can have many probable causes, but most generally results from otologic disorders "the same conditions that cause hearing loss. The commonest cause is noise induced hearing difficulties, caused by exposure to inappropriate or loud noises. Tinnitus, together with unexpected onset hearing loss, may have no clear external cause. Ototoxic drugs could cause subjective tinnitus either secondary to hearing difficulties or without deafness and may increase the damage done by getting exposed to serious noise, even at doses that are not in themselves ototoxic.
Subjective tinnitus is also a side effect of some medications, such as aspirin, and may also result from an abnormally low level of serotonin activity. It is also a classical side effect of quinidine, a Class IA anti-arrhythmic. Over 260 medications have been reported to cause tinnitus as a byproduct. In many cases , however , no underlying physical cause can be identified.
Tinnitus can also occur thanks to the discontinuation of therapeutic shots of benzodiazepines as an element of the benzodiazepine withdrawal syndrome. It can occasionally be a long symptom from benzodiazepine withdrawal and persist for many months.
